Immunizations

 

Effective January 1, 2015, ALL non-medical waivers (religious or philosophical) must be issued from the local health department.  Contact your local health department for more information.

 

All incoming freshmen are required to have a physical examination before school begins in August. The physical form is sent to the parents/guardians and must be returned to the attendance office in August.  

 

Wayne County Health Department will notify the attendance office if a student has not presented

1) acceptable evidence of the required immunization, or

2) a medical statement indicating a contraindication to the immunization, or

3) a waiver indicating a religious or other objection to immunization.  

 

Students will be subject to exclusion from school if no evidence of immunization or waiver is presented to the attendance office.
